This model is Jasper Breur 1/35th scale model of the Leopard 1V. The Leopard 1V was the upgraded Leopard 1 as used by the Dutch Army. It comes with a number of changes that are not found on other versions of the Leopard 1. Most notably Dutch-type smoke grenade launchers and an armoured cover for a planned IR sight. These modifications were kit bashed from the Revell Leopard 1 or scratch built onto the base kit: An old Revell Leopard 1A1A1. Decals were designed and printed by Jasper Breur to depict a vehicle seen in a picture I found.
