It s a M925 A1 with a big pusher and a up armoured cargo bed.

Wheels are a old MR Model (Ger) reference, and the plate are styren sheets.

Any on-line supplier has them; Great Models, Sprue Brothers, Squadron, etc., etc...
or order them direct from Real Model, CAM Designs, CMK, etc., etc.....
Quite a few companies make resin M900 series 5 ton replacement wheels.
