The only ones I know of were made by DML/Dragon. Unfortunately, both are out of production.
Perhaps an eBay search is in order...
EDIT: Ooops, I forgot the second part of your question. You can use this set on any of the Tamiya M4A3 kits. I would suggest the later one - "Frontline Breakthrough" kit # 250.
BTW, here's an article concerning the AFV kit: M4 Sherman HVSS Suspension by Terry Ashley

Dragon does a HVSS set as well, a lot easier to put together and it is also much cheaper D3814.
The Dragon kit will be the cheapest option.
Otherwise the next cheapest option will be:
Dragon's HVSS kit,
plus the Italeri Marine M4A3 kit's Hull or any Tamiya M4A3 Hull (M4A3E2?),
and Italeri's M4A1 kit's Turret.
You will still need to make / get a muzzle break as well.
Hasegawa make a 1/72 (Almost) kit of the M4A3E8 (quite nice)
Hope this helps!

Savage,
Do you happen to have an extra one that I could buy from you?

I have heard and seen that AFV Club makes a nice HVSS conversion set, but I have not built it.
Like has been said, if you don't go for the Dragon/DML set, you could use a Tamiya or Italeri M4A3 hull and an aftermarket or Italeri turret (from the M4A1).
Jordi Rubio makes a fabulous 76mm barrel for Shermans that comes with the option of the muzzle brake or protective collar. It is almost a must-use for the Dragon/DML kits, because their barrels are noticably too short. It's also nice for the Italeri M4A1, as it has a barrel with a "step" in it that doesn't belong there (the kit was apparently based on an Italian Army vehicle, which had its barrel cut off and spliced back on with a collar.)

MP Models used to make a set of HVSS tracks that at one time were the best things going. They company is out of business and the quality has been surpassed by DML and AFV Club's HVSS. They are still available in clearance bins and at model shows, usually real cheap. Less than $5.
The OOP DML M4A3(76)W HVSS is the only kit that comes with the single pin track shown in the photo. The rest of the sets come with the newer chevron style double pin tracks. None of the Tamiya kits come with the late war 76mm turret, just the OOP DML kits and the Italeri M4A1.
What you really want to look for is the Resicast M4A3 hull with 76mm turret and soft armor (#35-112). This conversion set is made for the Tamiya kit. The upper hull is made out of resin and has all the sand bag and metal bar racks molded on. Looks extremely similar to your Osprey photo. May be OOP, but out there somewhere collecting dust on someone's shelf.

Oooo, Rob. You've been really doing your homework on Shermans. Got a project in the works?

I think the DML M50 Israeli Sherman is still in production That has the E8 suspension but it comes with the T80 chevron tracks You can mate that suspension to the Italeri M4A3 and get an AM T23 turret #:-) (++)
