My guess is modern, Afghanistan, and vehicle doesn't matter, although intuition would be a Stryker.
If so, the only factory painted MultiCam helmets I've seen are Ops Core. The CVC could be painted by a crew member any way he wishes. Usually the pattern doesn't look as good as the original, but at least they're trying. I'd suggest something personalized. Skulls, spades, eight balls, you name it, it's been done.
Spraying green over sand through a fishnet stocking seems to be a favorite in my community...
