Plus model has a conversion you can use. Its made for the Italeri Mercedes L3000.

Ivan - Plus Models makes both a Blitz 4x4 ambulance as a full kit and a 4x4 conversion kit that will work on just about any Blitz or Mercedes model out there. The kits may be a bit hard to find but they are out there!
I too was bitten several years ago by the need for a 4x4 Opel Blitz truck. Do not know why the hobby industry has been so remiss in providing a cure for this problem.

Myself, I took a different and cheaper route:
I suggest you start by locating (on eBay perhaps) an inexpensive Italeri Chevrolet CMP 15-CWT Truck model, Kit #233.
This wartime vehicle was built by Chevrolet of Canada which was owned by General Motors USA, a company that before Hitler stepped in, also happened to own Opel of Germany.
It is reasonable to think that the parts produced by Opel during the war were based on designs and concepts developed jointly by Opel and their parent company prior to the war..
The #233 kit will provide you with a very reasonable GM/Opel 4x4 transfer case, nice banjo style front + rear differentials and axles plus a set of well detailed drive shafts necessary for building the Blitz 4x4 in 1/35th scale.

Great info, as always. I currently have two 3T 4x2 Opel Blitz kits. A Tamiya 35291, and a Italeri 216 I found at a garage sale (a few parts plus wheels/tires missing).
I ordered the Plus Models 4x4 conversion #156 and can hopefully modify it well enough to fit either kit.

Now the body modifications are another matter: The front fenders must be cut off and moved both downward below the line of the bottom edge of the bonnet side skirts and also moved a bit to the rear.
Those same bonnet side skirts must be rebuilt and the old louvers removed, sanded down very thin and reapplied to the new side skirts.
Also that front bumper must be modified to work with the now lower fenders now mounted further to the rear.

That's the one! Even the cover photo has much to say about how the driveline is put together.
You will note there that the Opel uses an oval shaped differential not round. The CMP has round, however that only requires that you make a thick saw cut down thru the differential pumpkin to produce the necessary appearance. Interestingly the Blitz had a narrower wheel track than the CMP so this one cut accomplishes two goals in one shot.
